To register for Clinical Study Days 15, please submit payment and complete the form below.

Standard Registration (After November 1st): $330
Student Registration (For those 26 years old and younger.  A student photo ID required at event): $100

CSD is nearly at capacity with a limited number of seats still available. The event will likely sell out and new registrations will not be possible at the door.

Permission for our use of the NPAP building as the Clinical Study Days 15 conference site requires that those who enter the NPAP building have received vaccination against COVID.  We will be checking for proof of vaccination at the time of registration.  Proof will be required for you to participate.  A photo of vaccine confirmation is acceptable proof.

In addition to this requirement, we strongly recommend (but are not requiring):

That you receive the most up-to-date COVID vaccination booster as appropriate
That you test for COVID prior to CSD15 each day
That you wear a mask during CSD15.

If you have registered and do not meet the vaccination requirement, you will have your registration refunded.

Applications for an exemption for the vaccination requirement based on a medical contraindication to COVID vaccination will be reviewed and considered, provided that they meet the “essential elements” for a medical contraindication exemption for vaccination requirement.  The essential elements for consideration for an exemption can be found at the bottom of this message.  (These recommendations are directly based on the Center for Disease Control guidelines, which can be found in the paragraphs “Medical Contraindication to COVID-19 Vaccination” on the following CDC website:
